Team Registration Form

Important

This form needs to be filled in by every team member. Teams can be up to 10 people.

• If you've got a whole team: make sure all members fill in a copy of the form and include all team member names in the "team members" field.
• If you've only got part of your team: make sure the members you have fill in a copy of the form and include confirmed member names. As you get more members, ask them to fill in the form and include your name.
• If you're still looking for team members: you can leave the team members field blank. As you find members, get them to put your name in the member field. If you don’t recruit a team, don’t worry – we can put you in a team of like-minded people.
